Can I mount a USB pen drive formatted in e.g. ext4 and allow ESXi to read/write in it without creating datastore, etc?
(I'm just scripting hot backups that's all)

No, ESXi won't recognize that. Your best bet is to attach that USB drive to a workstation host and use something like Veeam to back up the data to that drive as a repository.
