Could you please observe the events at vSphere and see if the App Package disk is attached to Virtual Machine at the time of logon?

On App Volume Manager Server: Please Navigate to "C:\Program Files (x86)\CloudVolumes\Manager\log" and open the latest ProductionXXXXXX.log file and go to end of the file and search upwards for "ERROR" and "500" or with the name of the agent machine. If you see an exception, locate the beginning of the exception which will give information about the cause of this issue.
