Microsoft Outlook iOS App Automatic Configuration Issues

We have an Exchange On-Prem infrastructure and despite following this guide we're unable to pre-populate the required fields for MS Outlook iOS.

The only field that pre-populates is the user email address.

This occurs on both employee owned and corporate devices.

Any ideas?
