T-shirt dropdown feature which you are asking for will be available on version certified by servicenow(will be available soon).It is not present in current version which you are using.

I will update VMware solution exchange as well with same copy of application and other supporting documents soon.

Where can I find installation/configuration documentation similar to that provided by V5 of the plugin?

You should see option for documents on store itself. (I can not cross verify as I don't have access there.)
If still you see issues, please use VMTN thread where I have uploaded V5.1 document. VMware vRealize Automation ITSM Application V5.1.0 (ServiceNow Certified)

However, I am not seeing the T-Shirt Size drop down working in the vRA User Portal catalog items.
We are using the VMware preferred method using the component profile called "size".
Can you please outline your supported T-Shirt Size drop down method along with some examples.

Remove it from exclusion list system property, update the record and run import again to reflect changes.

This worked really well, we now have the T-Shirt drop down list working nicely and we have hidden the cpu, memory & storage fields.
I do have 2 issues with the install documentation though:
1. It is not available from the ServiceNow Store
2. It does not reflect the new ServiceNow Store install process (still only references the Update Set process)
