Multi Site ID Manager - SQL Always On Config

Can anyone describe how they've implemented an external SQL database for multi-site ID Manager?

Reading the white paper 'multi site identity manager' and 'horizon 7 enterprise ref architecture' white papers, it suggests you use a SQL availability group for the Identity manager database, to allow for multi-subnet failover between datacenters.

Has anyone actually implemented this?  If so, how was your availability group configured?

They suggest using 4 SQL nodes, to allow for automatic failover within each site, and manual failover between datacenters.  Makes sense.

Admittedly I'm a bit of a SQL novice, but I can't see how this is actually possible - a 2014 availability group only allows up to 3 synchronous replicas (including the primary), and synchronous replicas are required for automatic failover.

Unless they're suggesting to configure auto-failover in the primary site (using 2 synchronous replicas), and 2 asynchronous replicas in the failover site?  In which case, automatic failover between nodes in the failover site wouldn't be possible?

Any SQL experts care to share how they configure availability groups for ID manager and App Volumes databases, in a multi-site (2 datacenter) horizon enterprise environment?
