I have seen it several times before, however I am unaware of any "Solution."
A work around I have used is to change from the SAN VCB to a NBD (Network Based) VCB. The network VCB seems to request the data from the ESX Server your VM is on, thus negating the VCB Proxy's need for SCSI IO to the Datastore.
Are you using Powerpath? only versions 5.0, 5.1 and 5.2 are supported. Also, check your SAN health. Are you using 50micron cables? Are they plugged in all the way?
You can also run the vcbSanDbg utility to see if any errors appear.
