For the vCenter license you will need to purchase 'VMware vCenter Server Standard' since you have more than 3 ESX hosts.

As far as the ESX licenses it depends on what features you would like... http://www.vmware.com/products/vsphere/buy/editions_comparison.html

What features are you looking for VMotion, HA, DRS, vNetwork Distributed Switch, etc?

If you're talking about which bundle(s), it depends, entirely, on what you'll want to be able to do with the environment. Do you want vMotion, HA, DRS, etc?

Look at the VMware site sections for SMB as well as for Mid-Size & Enterprise Business... At my last job, we purchased the Enterprise license for three hosts, plus the vCenter Server (part of the Foundation package)...

ESX/ESXi 4 licenses you per host server (previously it was per processor socket in the host)...

Look at the different editions, to figure out which aligns most with your needs (better to get a little more than less than you need in my opinion). Then, reach out to a VMware Premier partner in your area to gain more detailed advice. If you don't already have a SAN solution, you'll need to get one. I would opt for iSCSI there, but that's just my personal preference (there's no longer enough of a speed difference between properly configured iSCSI and fiber solutions).

It depends by your needs.

Which function do you want?

If you do not have a shared storage, you can start from the Essential bundle that cover 3 hosts and the vCenter license (there is a 50% discount promo).
