I have a 3 host cluster I am attempting to upgrade. Update Manager was timing out on remediating the hosts so I connected a USB bootable ISO to the machine. It went through it's boot sequence and made it to the ISO. It goes through and begins installing ESXi, makes it to the end where it installs 'nfsclient' and then kicks out an error: There was a problem with the Network Device Specified on the command line. Error: No NIC found with MAC Address: xx:xx:xx:xx:xx:xx. The address it is referring to is the FCoE QLogic card installed. This is the card that was running 4.1 traffic with no problem.
The card itself is in fine condition and even visible as a card in the UEFI system info.
Anyone seen this?
The Qlogic 10Gb CNA adapter for x3650M3 (42C1800) requires driver qlge-1.0.0.47 for ESXi 5.0. It's in the compatibility guide.
Search for IBM part number 42C1800 in VMware's HCL: http://www.vmware.com/go/hcl
If you're willing to do a fresh install of ESXi 5.0 instead of upgrade, you can also use the ESXi 5.0 Driver Rollup 2 ISO to get that driver installed during install.

The release notes for that ISO contain a list of drivers included with it, one of which is qlge1.0.0.47.
