Vsphere 5.5, EVA8400(HSV450), Microsoft Cluster 2008 R2 PROBLEM!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

I am looking for some expert advise about HP EVA storage on VMware, I have configuration like below

ESXi 5.5.
VMFS 3.4 version
Microsoft Cluster service with 2008 R2
and Storage i EVA8400


The problem is when i attach RDM or VMDK to MS cluster VM's, its not formating and giving an issue and I found following errors in ESXi

Storage policy for disk '/vmfs/volumes/523194e2-b4ea10a8-f837-0017a477fc94/LONFAPP04/LONFAPP04_1.vmdk' not specified.

but strange thing is, sometime format works and sometime won't work

can someone identify root cause of issue from above error messages?
