I'm periodically having connectivity issues between an ESX host and multiple storage volumes, which are generating these errors:
"Lost access to volume due to
connectivity issues. Recovery attempt is in
progress and outcome will be reported shortly.
info"
What are most likely causes for this? How can I prevent this by insuring proper redundancy and failover in the various parts of my storage configuration?

Which kind of storage do you use?
FC SAN?
iSCSI?
NFS?
Lokal SCSI?
Please be more descriptive about your storage setup.

This is Fibre Channel. There are 2 Fibre Channel HBA's per host and 3 targets per HBA. Path selection policy is most recently used. Let me know if any other info is helpful
Are you facing any outages or problems with the running guests or do you get "only" that message?

I suggest you to check your SAN switches log as well. Could be some problem caused from the switches.

It turned out the 2 storage controllers on the storage system had rebooted suddenly for some reason. Not sure why yet.

Hmm did you do a code upgrade for this to happen ? seems very strange for two processors to reboot. What array do you have ?
