There are different ways to tackle this one.
You can leverage this article as a base : How to use and extend the workflow "Request a catalog item" from the vCAC plug-in for vCO
And build your own workflow determining the parameter to pass to this workflow based on a small, medium large drop down.
Another way I am less familiar with is to use IaaS build profiles.
