Normally this should be one:

Is this a custom dashboard or OOTB one? And you only have one vCenter attached to this vROps instance?

This is custom dashboard.

There are two vcentre server

1)node vcentre server

2)management controller vcentre server

but respective host is belongs to only for management controller vcentre server .

Note- this issue only for hosts derived from management controller vcentre server , however hosts derived from node vcentre server is showing correct (i.e.only one collection status and state)

Can you try to stop and start the vCenter adapter instance collection for the management controller vCenter server?

You can find this under Solutions --> Configuration --> VMware vSphere --> select the vCenter adapter instance.

This will happen if you have more than one management packs collecting from the same object. The below is for one of my hosts. It is collecting for the vcenter adapter, vsan adapter and two unicorn adapters. When i create the different adapters i identify them differently

vcenter xxxxxx

vcenter xxxxxx (vSAN)

etc

That way when you hover over the collection state you can tell the difference
