cannot add disk in vCenter VM using vRA8.1 Cloud Assembly

After creating a VM using vRA8.1 on vCenter, I ran add disk from the action menu, but the message below appears and I cannot create it. What could be the cause?

Here is a workflow i wrote just for this.  It basically will add disk to a VM and then using the resource action copy a powershell into a VM and format the newly added disk.  The contents are not packaged correctly (naming standards) but you should be able to find the workflow.

Thank you for your response. When I tried to add it to a resource action using the "NGIC - Add Disk to VM" in package I received, I was unable to use it with the following error message.

However, when I defined the Add Disk workflow in the vRO standard as a resource action, it seems that I was able to add a disk.

But I still don't understand why the standard resource action Add Disk action fails.
