I use vCenter and vsphere 5.5.

Is it possible to make VM template which include application, database and make VM which application, database work fine from that VM template ?

I think application and database application includes uniq information like host name, and I wonder it work fine if VM is made from such VM template.

It's possible to create the template with your application and installed. However, I would leverage chef, puppet, or some type of powershell/bat script to modify all the unique values that will setup the rest.
