Have you tried reinstalling the agent on the parent? 
Have you explored the disposable disk to see what is filling up?
1:  I think this answer is going to be dependent on different environments you will need to test in yours 2:  We have had no problems with persona.

Is this all through the connection broker or do you have load balancers involved? Is this a floating pool or a dedicated pool? 
You may want to input the credentials again under View configuration, servers, vCenter server on the View admin page.

Not sure why it would remove the service but there is an alterative way of doing this.   Instead of upgrading the existing broker OS you could built a new 2012 R2 server and simply join it into the replica group.   Once you verify everything is working correctly on the 2012 server you can retire the 2008 broker.
That is normal behavior if the pools are using the same snapshot and replica datastore.   It's been like that for awhile.

Replicas are removed by View composer when there are no longer any clones dependent on the use of that replica.    From time to time they will not get removed properly and when that happens there is a KB, VMware KB: Finding and removing unused replica virtual machines in the VMware Horizon View, that can assist in the proper removal of the replica.

You are correct in that if you use a replica datastore or the pools share datastores then they should be able to share a replica if they are using the same snapshot.    I would use the View Composer details view on the inventory tab of the pools to determine if maybe a single machine is using an older snap.

In addition to what Joshua said can you also list what versions of vSphere, esxi, and View you are using?
