Provision User Data Folder

So have another new problem.  Running Horizon 1.5.  Have created a new COS, waited 30 minutes, entitled group.  Get an error that the user couldn't be provisioned.  The horizon.log on service-va says "invalid request: missing required attribute: name".  In the Workspace admin page I see an error on the user saying "Cannot retrieve COS, Error Provisioning User to Data".  this is a fresh install.  I even tried adding the name attribute to user attributes and then doing a directory sync, thinking that was the name attribute it was referring to, didn't help.  Any ideas?  thanks

Do you see any errors in the log file in service-va - /opt/vmware/horizon/horizoninstance/logs/horizon.log?

Yes, I included that in my original post... The horizon.log on service-va says "invalid request: missing required attribute: name".

Sorry, should have read your earlier post more carefully.

Can you please hit Retry in admin UI for provisioning this user? Then, send the logs to me from service-va and data-va?

service-va: /opt/vmware/horizon/horizoninstance/logs/horizon.log

data-va: /opt/zimbra/log/mailbox.log
