One of my users called with a report that he could not enroll his brand new iPhone. The Intelligent Hub opens up and immediately asks him for a username and password (the header of the screen says:

Enter Workspace ONE Hub enrollment credentials to access device status details.
The expected behavior is for it to ask for an Email Address or Server, and offer to scan a QR code, like in this screenshot:
Can anyone tell me any reasons that this would be happening? FWIW, we just upgraded Workspace ONE to version 20.1.0.15, but other devices are enrolling just fine. It's just this one that is strangely asking for Authentication creds. Also worth noting is that the user states that when they got their new phone, they did the "bump next to old phone" method of doing the iCloud Restore. I thought that might be the issue, so we removed the Intelligent Hub app and rebooted the phone and reinstalled the Hub and tried again. I have also ensured that there are not Management Profiles.
I have seen the Authentication screen randomly appear on some devices during enrollment. We are on 1907 on-prem and currently struggling with high IIS work process issue. Airwatch pool sometimes consumes high CPU and memory and that breaks the communication between hub and DS. Rebooting the servers re-establishes the communication but it is very frustrating to repeat the whole reboot process. I thought Authentication screen might be leftover impacts of IIS issues. It usually goes back to original Email or server address screen automatically. Sometimes rebooting the device, kill and relaunch hub, changing the network fixes the enrollment page.

Although this issue resolved for a while, it has returned again.
When transferring from one iPhone to an upgrade, Airwatch is not installing correctly. As before, it installs normally, but then when attempting to register it, the options are incorrect. On opening the Hub, we are supposed to enter the user’s Safran email to begin registration. The Airwatch app is not giving this option.
Normal: Enter server email address, and then enter console supplied TOKEN
Currently: Option is available to enter username/password – this is likely for non enterprise versions?
Option is available to enter TOKEN – but no option to enter server email address
The attempts I tried last time do not work now. Do we have suggestions to resolve this?
