Unable to install ESXi 6.5, 'Could not stat device'

Brand new setup of UCS B200 M5 blades with ESXi 6.5 to be installed.

Four out of six install without any problems but the following two can't complete the installation, they always fail with a couple of different errors:

'Could not Stat Device' or 'Unable to get Device' (see attatched images).

Boot volume is a pair of SD Card, FlexFlash.

I've tried all the classic fixes such as Scrubbing and formatting the SD Cards, resetting the controller and so on.

The odd thing is that if I try using a ESXi 6.0 image then the installation works just fine.

That was spot on, just upgraded firmware and successfully installed 6.5 on all blades.
