Studying a Performance Problem: Gap between DAVG/rd and GAVG/rd

I've got a VM that's performing poorly on IO.  I dig in with esxtop on the datastores (all NFS mounted) and I see that there's a surprising gap between DAVG/rd and GAVG/rd:  like 15 vs 56.  It's an NFS mounted datastore, so there is no information in esxtop about KAVG/rd (which I was told should account for that gap between DAVG and GAVG). 

Any suggestions on where to start looking?

You say there is a difference, but whats the numbers?  Look at ESXTOP - Yellow Bricks | Yellow Bricks  for a good reference

For example, I might see a DAVG/rd of 15 and a GAVG/rd of 56.
