Im planning to upgrade my vcenter environment from 6.0.0 (Build 3634793) to latest 6.7u2a.
currently i have 3 servers :
1.DB - for SQL running on windows 2012r2
2.PSC - Platform Services Controller running on windows 2012r2
3.VC - vCenter running on windows 2012r2
I want to remove this mess and create one VMware vCenter Server Appliance on Server 2016..(maybe later i'll create another one for HA.)
whats is the proper way to Upgrade my environment?
of course that later ill update my ESXi hosts one by one to 6.7u2 also..

The VCSA is a Appliance and runs on PhotonOS (The linux distri from VMware) so there is no Windows OS envolved. What you have to checked
You have to find the latest PSC migrate howto. Problem is that the supportet ways have changed over time. I remember that the external PSC was the way to go for quite some time... but not anymore.
Question:
Is this a larger installation? Is vDS or VDI in the game? Is there only this one vCenter or multible(linked mode?)
Just a fresh new install is not an option?

Migrate the VC and PSC to 6.7U2 appliance. this will create 2 VM's and SQL will no longer be needed as 6.5 onwards we have vPostges DB.
Once you migrated to 6.7 U2, use the converge tool to decommission the external PSC.
Once you have the embedded VCSA, you can configure the VCHA for vCenter server high availability.
Inside wizard of the VCSA setup, 3rd option is Migration. It's used only for Migration from windows-based vCenter Server to VCSA (Linux-based)
Your SQL database will migrate to vPostgres database and data will merge inside the New DB into the VCSA.
Nothing will happened to your vCenter managed objects like DVS. no need to worry
Remember: Please take a full backup of your vCenter VMs and also its database before migration procedure

But what about PSC ? Do I need to upgrade or migrate PSC Server ? and how do i decommit this server after upgrade \ migrate ?

Please refer my previous update. You need migrate the PSC first and once VC & PSC migrated to appliance.. you can perform the convergence to decommission the external PSC.
If you choose Embedded (It's recommended because of after vSphere 6.7, external mode may be deprecated) then it will move PSC data, roles and services to the new VCSA and then do it for vCenter services.

There is a contradiction in what you wrote: "VMware vCenter Server Appliance on Server 2016" is an appliance... you can't install on an operating system.
It is not possible to migrate from external PSC to embedded PSC. But this is something we're working on, so my recommendation is to upgrade to 6.7 and keep your external PSC rather than starting over with a new vCenter server. The goal is to provide a migration path in the future.
The only way to do this currently is to deploy a new vCenter server.
