PowerCLI script to gather vCenter info.

I am looking for a PowerCLI script that is able to gather all the info in the attached excel sheet. I would like it to produce a csv file with this info. Unfortunately I am a newbie when it comes to PowerCLI scripting. I have been able to gather scripts that get most of the info but I have not been able to put it all into one report.

Just a follow up, I know not all the info is possible to grab like the Application name. If the script grabbed 90% of what I am looking for I would be extremely happy.

You can get all the info in csv format from RVTools.
