OS X 10.11.3 "unable to retrieve kernel symbols" after reboot

Hello All,

I seem to be unable to reboot my computer without then encountering the following 2 errors

"unable to retrieve kernel symbols"

and

"failed to initialize monitor device"

If I re-install the most recent OS update it again works, until I reboot.

Any ideas?

A few more details.

Using Fusion Version 8.1.0

I do have a separate encrypted partition on which the images live. I'm running Arch linux in the guest.

I am able to reboot the VMs as much as I like after re-installing the most recent OS upgrade but as soon as I reboot the host the "kernel symbols" seem to disappear until I re-install the most recent OS update.

Any thoughts and or data needed for possible solutions?

Which exact OS update package are you reinstalling, and how are you reinstalling it?

To collect much more useful information, launch Terminal and run the following command, both when the system is in the failing state (before installing the update) and once things are working (after installing the update):

   uname -a ; ls -l /mach_kernel /System/Library/Kernels

Paste the output from the failing and working state back into a post here and I'll see if I can figure out what's going wrong.

I'm a bit confused here.  How exactly are you going about reinstalling osxupd10.11.3.pkg?  Does it not require a reboot as soon as the installation completes, or are the kext versions changing without the system needing any reboot at all?  Or does it work after one reboot post-update and then fail after the next reboot?

Does your /var/log/Sytem.log have any complaints about failures to mount or work on the Recovery HD while the update is installed or shortly afterwards?

Clearly you are ending up with a mismatch between the running kernel and the kexts on disk, but I've not seen this particular pattern before and can't really understand how it's happening.  Anything else unusual about your hardware and/or OS boot configuration that might help?  Are you using any RAID system (hardware or software)?

Correct, it works after post-update reboot but not after any future reboots.

I do see instances of this if I run  `syslog | grep covery`

I initially chopped up my disk to install linux natively using rEFInd. That worked, kind of, had some video card/sleep/wake issues though.

I now have a hatcheted 500GB drive
249.68 for my primary Mac OS X drive

650 MB for Recovery HD

209.7 Linux HFS+ ESP (may have had something to do with the rEFInd)

223.24 extended encrypted space

25GB for a kali linux playground.

While I'd like to just re-install I swapped out my internal optical drive for an hd caddy and my mid-2010 model doesn't seem to allow booting from an external drive.

What's the mount point for your "extended encrypted space" (assuming it's HFS+)?  Is your Kali playground only used when booting Linux natively on the host?

I'd be interested to see a bit more context and the kextd logs, such as by collecting the output of:

   grep -C5 '\(kextd\|Recovery\)' /var/log/system.log

(Feel free to scrub out any identifying information in the output if you wish.)

I suspect kextd will be bitterly complaining about something there... or will at least tell us a bit more about what it's doing.  It *might* be that you are out of disk space on Recovery HD, although it'd be a mighty strange failure mode...

On my most recent reboot I pulled up the boot menu. It seemed defaulted on the Recovery HD.

I switched it to the regular mac partition and VMWare is working without issue after secondary reboot.

Thanks Darius for the tip on the Recovery HD.
