I'd go for a yes.
For company just fill in "none" or just about anything else...
This VMware site is tuned for corporate communications and somehow they did not manage to consider that a private person registering for a personal license does not have a company 
really not sure what those developers had been smoking, but it must have been very good stuff.
Not sure about the phone number, but it isn't like they are going to call you ... I"d put in anything that is accepted. I'm surprised it is even required?
