vRA failed to switch root at install

I am working to install vRA and the vRealize Automation vm is stuck at "failed to start Switch Root", and the VM appliance is stuck in emergency mode.

What version and what steps are you taking to perform the install?

Are there any messages in the installation logs rather than just the VM console?

I am facing the same issue:

failed to start switch root. see 'systemctl status initrd-switch-root.service' for details. I am using vRA 8.8 for installation. 

vRLCM throws error showing Guest Tools are not started, checking in VM console gives the error -- failed to start switch root. see 'systemctl status initrd-switch-root.service'
