ESX 3.5 Upgrade 4.0 iSCSI SAN Disks not visible

We have an Infrastructure with one VC4 and two clusters with 2x 3.5 Hosts and 2x 4.0 Hosts. Both cluster have connection to a netapp FAS 250. After we upgrade the VC the 3.5 Hosts can´t see the disk of the second LUN and the 4.0 Hosts see the path to both LUNs but don´t show any disk. VC says that state of path from the 4.0 Hosts are failed. After many hours reading documentation and whitepapers from Netapp and VMware i don´t find any solution for our Problem. On 3.5 Hosts i can connect the disk on the second LUN if i have a single VC session on the host but not from a VC Session.

What can we do?
