Why is rescan vmfs failing?

Rescan vmfs fails with the following error message:

Task Completed : haTask-ha-host-vim.host.StorageSystem.rescanAllHba-15615

Task Created : haTask-ha-host-vim.host.StorageSystem.rescanVmfs-15619

VmkCtl RescanVmfs() failed: Unable to scan adapters for VMFS

Task Completed : haTask-ha-host-vim.host.StorageSystem.rescanVmfs-15619

How does one debug this issue further? Why is rescan vmfs failing?

Please note: I am rescanning for LUN snapshots with LVM settings EnableResignature = 1 and DisallowSnapShot = 1.

This is on ESX 3.5, update 1 (VMware ESX Server 3.5.0 build-82663)

Is there a rescan patch that needs to be applied here?

vmkfstools -V gives

VmkfsLib_ScanAdapter: Failed to do a volume cache rescan Status: 0xbad0003

Failed scanning adapter 'vmfs-only' : Not found

Error: Opaque service console status
