Can you also send the output of:

cat /proc/bus/usb/devices

What kind of system are you using? (Make and Model)

Is your CD/DVD IDE or SCSI?

Hi

This is a Sun X4200 with 2 x dual core AMD CPU's and 8GB or memory

it has 2 x internal HD's with LSI 1064 SCSI controllers. it also has an IDE CD drive, and no floppy drive

USB used for keyboard. The system has been working without issue, however issues ,may have started when I mounteed the virtual floppy for device drivers?

Maybe your host have a remote management controller with virtuall devices (cd, floppy), or you have a usb stick connected?

Anyway I think this isn't any critical issue, IMHO.

No USB devices unless someone else plugged one in when I was not there (could happen - its in a lab)

The i/O error caused a journaling failure - not on site so will capture / post specific error later, but after a few days it leaves the box unusable and needs a reboot

It is for a PoC so not a real issue until they start testing VDI

It may have started when I connected a virtual floppy for driver installations... however it references sdc - which does not exist

Thanks
