Multipathing problem with QLA4022

Hi, first at everything excuse my poor english.

I have two QLA4022 with redundant paths in my ESX 3.0.1 server, one of them are conected to a switch and the other one are conected to another different switch and they work perfectly.

When anyone of the path go down by a moment the path changes to dead mode and communications change to the path (the other iSCSI adapter). But when the dead path go up again, the ESX continuous showing that path in dead mode unless Rescan SAN is pressed.

All configurations of multipathing has been proven (Fixed and MRU).

This is not what I understand by failover.

Can you post here the output from

esxcfg-mpath -l

What storage are you using?

No, I can't because the problem happens in a customer and i have no access now.

The storage is NetAPP.
