Running ESX 3.0.1 added an Emulex dual port FC HBA to the blade and rebooted the box. The Emulex BIOS is disable, which is consistent with the other blades running HBAs - these servers had ESX installed with the HBA already present and work fine.
When the box boots, I get the following error in the startup log:
Feb 12 16:51:03 esx-blp45 vmkload_mod: Using /usr/lib/vmware/vmkmod/lpfcdd_732.o
Feb 12 16:51:03 esx-blp45 vmkload_mod: Failed to load module: Invalid argument
When the server boots the following message appears:
Cpu6:1088)Mod:490:Initilization for lpfcdd_732failed with -19
Cpu1:1037)Mod:490:Initilization for lpfcdd_732failed with -19
I have downloaded and installed the following: elxvmwarecorekit-2.1a29-7.3.x (latest version for 3.0.1) and lputil, reboot the box. I am holding back on trying to configure the HBA until it loads properly.

Re-seating the HBA card resolved this issue.
