i have this issue and any suggestion will be helpfull.
i have IBM blades that connected to SAN storage. ESX 3.5 and ESXi 3.5 installed on 2 blades , we have VM's on the local disk when we started moving these virtual machine, from the local ESX
or ESXi disks to the shared storage disks, we start facing a very
critical and dangouruse problems as follows:

During
the copy or moving from the local disks to the shared disk, an error
message appeared, the error message was “ General Fault caused By file"
After pressing the OK
bottom the attached storage partition “ that we were copying the data to”
disappear from all the servers in the group.

After investigating what
was happed, we discover that the storage partition still available from the
storage adapter but with no VMFS file system, and *_All The Data on That
Partition Was Lost_*.

After the data lose, when
we try to add the storage shared partition, it says that the partition is blank
new, and we have to format it to add it again.
