Add extent - no device showing

VMWare ESX Server, 3.5.0, 110268, FibreCat SX, did esxcfg-rescan vmhba2 and a reboot

After extending the LUN successfuly for 4 times we cannot add the extent anymore.

The SCSI Target is showing a higher capacity than the storage device and it is not possible to see or add the additional space.

You cannot create another extent on the same LUN, because all 4 primary partition slots have been allocated.
