Hello, I assume you're using Update Manager 5.0, your source ESX host version is 5.0 and you want to update it to build 1254542 (Patch 06). If that's the case, please note that Host Upgrade path supports upgrades from pre-5.0 major version (4.0, 4.1) to 5.0.

Does your Update Manager system get patches from online portal? In which case, you can patch base ESXi 5.0 host with Patch 06 via Host Patching baseline (not via Host Upgrade baseline).

The host is HP.  I had to create a new image to include the new version of PowerPath (as well).

Looks like you are using an entire ESXi installer file for a patch upgrade.

Go to update manager & scan the ESXi host against default baselines "Critical & Non critical host patches". No need to use ISO for patching.
