Here is a nice blog for the best practice for p2v. http://ituda.com/vmware-p2v-converter-best-practices/

Troubleshooting Guide : http://www.vmwareminds.com/troubleshooting-while-doing-physical-to-virtual-conversion-p2v/

Conversion tool should be download from here. : https://my.vmware.com/group/vmware/evalcenter?p=converter

i always use vmware convertor for this. here you can connect to the physical server and move it to esxi server or vsphere environment

what physical os is it?

some Os will need a earlier version from vmware convertor like example windows 2000

(P2V Windows 2000 server – vSphere 5.1 | VDS Systems Blog Page)
