I know there's a couple "official" guides out there for Windows 7 optimization, but how much of the guide do folks follow? Can I get away with not doing all of it?
You don't have to do all of it as some of the items might not be appropriate for your environment. I would go through the guide and do as much as possible. There is also Microsoft tool that can assist with optimization
