P2V Consolidation Planning

Can someone point me to a thread, white paper, or document that shows everything that you want to think about and look at when deciding if a physical server is a good candidate for Virtualization. And a checklist of things that need to be done post Virtualization on Windows Servers.

The VIOPS site has some excellent information on P2V. The Proven Practice : Migration Methodology is a great resource.
