vmhba not detected via pass-through

I'm trying to setup nested ESXi on my lab and I have set pass-through for a couple QLogic 2562 HBAs. Installed 2 ESXi 5.1 as VM and added those HBAs as PCI to each VM. On host1 (esxi-lithium01) It only manage to find 1 device and on host2 (esxi-lithium02) it is showing as failed to initialize adapter and claimed 0 device. Appreciate any input/help as I have tried updating the driver but that doesn't seem to work.
