Couple of things you can check
First >> on the esxtop window check for the top line . example below. Check how many vcpus have been assigned and how many vms you have?
1:37:50pm up 1 day 4:00, 333 worlds, 2 VMs, 3 vCPUs; CPU load average: 0.02, 0.01, 0.01
Generally the ratio should be like 1pcpu to 3vcpus. but it again it depends on the ind of work load you have.
Second >> as stated above check for the cpu ready and also for co stop counters. if these values are on the higher side then bring down the vspus for the VM
Third >> Check the overall load on the esxi server
