... as an alternative to installing the Windows version of vCenter Server, you could take a look at the vCenter Server Appliance.

It's up to you. You can either use physical machine or virtual machine. If you are using physical or virtual machine you need 64 bit of windows server 2003 then you can start the installation of vCenter server if all the other prerequisites are fulfilled by you OR you can deploy vCenter appliance on one of your esxi host....
Windows version of vCenter 5.0 should be installed on a 64 bis OS, it can be installed on a Physical or Virtual (Vmware Recommended)

Create a VM with Windows 2008 R2 and install it there (recommended)
The VCenter Server Appliance built in database is recommended for only 5 hosts unless you have an Oracle DB or IBM DB2. It will not use a regular MS SQL database. The installed VCenter requires 2 cpus and 2008 R2 is recommended.
