Unable to create a new Volume

How i can add a new datastore without issu?

Every time when i add a new Volume i get this with a logout.

I'm having this exact issue trying to create a datastore with a Samsung 840 Evo 500GB SSD on a ProLiant ML310e Gen8 v2 server. I'm using ESXI 6.0 though. Any help on this is appreciated.

are you using embedded image , did you try direct from root through vc-client
