MegaRAID M5015 on ESXi 5.0.0

OK, this is a revisit of MegaRAID M5015 on ESXi 5.0...

We eventually got the MegaRAID Storage Manager working on ESXi 5.0 (as above).  We then upgraded ESX to ESXi 5.5 u2 (IBM / Lenovo build), and MSM broke again.  Many attempts at resolution failed (see ServeRAID M5015 on ESXi 5.5.0).  As a result of that, we came up with a couple of options:

1 - try it with 'vanilla' ESX / Windows, to rule out any build / environmental issues

2 - checking the Lenovo website, support of our hardware (IBM x3550 M3) stopped with ESXi 5.5.0 u1 - so using the u2 build could be the issue (although I'd be surprised)

So, starting with the first option, I have a vanilla build of VMware ESXi 5.0.0 (build 469512).  I've tried MSM with the driver / firmware levels that worked originally on 5.0, and now it doesn't work...  I've tried an older version of the LSI provider, again no luck.  Versions tested:

Have also tried a slightly different version of the MegaRAID driver, the Avago website suggests going to the VMware website for the driver for ESXi 5.0 – link https://my.vmware.com/group/vmware/details?downloadGroup=DT-ESXI5X-LSI-SCSI-MEGARAID-SAS-66060600-1O... – so removed the Avago vib (Avago_bootbank_scsi-megaraid-sas_6.607.06.00-1OEM.500.0.0.472560) and replaced with the VMware vib (LSI_bootbank_scsi-megaraid-sas_6.606.06.00-1OEM.500.0.0.472560) - but still no good.

Have tried with the ESX and Windows firewall disabled, no luck.  Even in this case, if I telnet on ports 3071 / 5571 to the ESX server, there is no response, suggesting it's not listening.  Does anyone have any other suggestions?

There are other MegaRAID vibs on the box (below), could these be conflicting?
