I heard it is possible to change CPU counts and memory without having CPU hot add/remove enabled through CLI. Is it possible to change the CPU and memory count without having to power off VM to enable the CPU and memory hot add? I work in a hospital environment and powering off a VM is not possible to enable these functions.

You can enable to Hot Add feature via power-cli but you will need to have have a reboot in-order to reload the new configuration.

I think, this is impossible, even if you can add it to your VMX file, you need to reload that.
If your virtual machines are supporting hot-add, enable the feature on them.
if the feature is disabled, as mentioned, you can't override that by using the CLI. To enable hot plug you'll need to power down the VM.
Is it possible to setup an alert trigger on when a VM is powered off to run CLI script to enable the hot adds?
sure... you'll probably have to create new alarm and trigger, but it should be easy enough. I too work in healthcare and although it's sometimes tough, there are downtime windows. We usually piggy back on those if there needs to be some maintenance done.
Also, don't know if you've done this already. Make sure you enable the feature on all your templates. This way when you deploy a new VM it's already hot plug enabled.
