ESXi 5 Boots to "VMware Hypervisor Recovery"

Hi All

I have a host which is running ESXi 5 and upon a scheduled reboot it only shows "VMware Hypervisor Recovery" at the top of the screen with no options at all.  I have ran a repair installation on the file system already and this hasn't resolved the issue. 

Has anyone seen this issue before?

its just a guess, is first boot device in BIOS is set properly? it should be the disk which has ESXi installation.

I managed to track this down to a dodgy USB pen drive where ESX was installed.  All sorted now, cheers.
