Lots of QLogic FCoE clues there - check the hardware/firmware/software of each element which connects your host to your datastores.

I'll also add that VMTN does not provide PSOD-analysis-as-a-service and if you really want to get to the bottom of it, you need to open a case with GSS and submit your evidence. They have the tools needed to fully analyze what's happened in these situations.
