Hi,
We have cluster contains three ESX server version 4 and we enable the vMotion. When we entered ESXs to the maintenance mode, two of them were working fine and VMs moved automatically.
But we faced an issue with one of the ESX, we tried to enter one of the ESX to the maintenance mode, but we could not because of one of the VMs which did not migrate automatically. It reaches the percentage of 78% and stopped. Also we tried to migrate it manually, but it failed to migrate.
vCenter is version 4.
Any idea how can we solve this issue?

This might be a configuration issue with the VM itself (see http://kb.vmware.com/kb/2001239). In case this does not solve the issue, please attach the VM's latest vmware.log file to a reply post.

Sorry, I can't really tell you what's causing the issue. You may however check whether the virtual CD-ROM drive is related. Set the virtual CD-ROM to "Client Device" and disconnect it.

Can you please search for Migration ID 1364807222146540 in vmkernel logs in both source and destination hosts?, it will give us more explanation why it has failed.
Run the below command on both ESX host and post the results.
#grep 1364807222146540 /var/log/vmkernel.log
If it returns nothing then try
zgrep 1364807222146540 /var/log/vmkernel* >> host1.log
Run the command on Destination host.
zgrep 1364807222146540 /var/log/vmkernel* >> host2.log
And upload host1 and host2 logs.
