I have some ESX 5.1 servers running the very base install build of 799733. Does anybody know if I am able to patch them straight up to the latest release of Update 3 (build 2323236) or do I need to install every previous patch first?

Assuming that your host is supported, you may directly update to the latest patch bundle.
Take a look at e.g. Are ESXi Patches Cumulative - VMware vSphere Blog for information about this topic. The last two screenshots show the update process.

Yes you can do ,
Download patch by using update manager , create base line and scan host, it will show if patch is applicable for the host.
