Both iSCSC initiator and target can ping each other. On configuration of iSCSI on host, after input iSCSI Server Address in Dynamic Discovery, in Static Discovery can see Target Name which on iSCSI Server provides.
But, can't scan out the iSCSI storage. After command esxcfg-resan, it report iSCSI connection is being marked "OFFLINE" by tail /var/log/vmkernel.
How can fix it?

Give us more details, what ESX version do you have, what storage model you have as a iscsi back end,
is it a new set up or it was working before and suddenly stops working
I had this same error with iSCSI storage, problem was that iSCSI storage hasn't support for vSphere and VMware in general

It is vSphere 4.0. iSCSI software is Starwind 5.2.
I used it before. and a few day did not use it. And late, vmkernel port reset on a single vSwitch which the others like Service Console and VMs are not here. As a nested host, two vShere VMs have their vmkernel ports on the vSwitch, too. The vSwich has a physical NIC directory to iSCSI server.
I guess if the 3 vmkernel port share a vSwitch cause the problem.

Can you do a #vmkping ipadd from the ESX console where ipadd is the IP of the storage target, is the correct IP address entered under the Dynamic Discovery tab ?
