VSS and VDR

I was just reading in

that VDR employs VSS to create (almost?) application-consistent backups. Is this in all cases or only when you have provisioned a data disk within the guest using MS ISCSI iniator?

It is only the case when a snapshot can take place on a disk. Example, it will not work with physical RDMs because a snapshot cannot be taken on that disk.

so, my question is: does it have to be a VHD (i.e. MS ISCSI initiated) or can it be a VMDK (vanilla vmware)?

Yes, it can be VMDK.
