It is currently only available through Amazon.com and comcol.nl for EMEA. if you know of any other online bookstores that we could use for distribution in those regions let me know and I am glad to send them an email and see what can be done.

Yes we planned for it, but unfortunately due to the fact that we are constraint in time it will not happen. hopefully we will have some help with formatting on the next version so that we can focus on the content.
