Adding a vCAC Host (6.0) - Embedded vCO - Which FQDN/IP and Username?

Which name/IP to you target for vCAC 6.0 when "Adding a vCAC host" in the embedded vCO?  The Identity VM, Appliance VM or IaaS VM (Windows)?  And is it root, SSO or AD login accordingly?  I think I have tried every possible combination without success:

Domain for NTLM Authentication:

NETBIOS Domain Name

specify the vCAC IaaS server (FQDN) when adding the vCAC server via the plugin.  Also remember the username needs to be in user@domain.local format
