1: Lets Assume in 3-5 UCS-Blade Server environment where we could place the Vcenter?
2: That can be VCenter as appliance or on top of Windows?
3: What are the recommendation and best Practice to place the Vcenter?
4: It must be on Separate Rack Mount Server or can be in any Blade inside the UCS?

A dedicated management cluster is the recommended place to put your vCenter server.
2: That can be VCenter as appliance or on top of Windows?
I will recommend you consider to use the version 6.5 and the appliance version.

Inside a management cluster with HA enabled and/or consider use the new vCenter 6.5 HA feature.

Like I said in my previous answers, I dedicated management cluster is the best place to put your vCenter, and is always recommended avoid single point of failure, so if you have a management cluster with nodes across differents racks, that will be the best place.

Your environment looks very small, based on number of hosts, so with that, the embedded database should be enough.
